NOVEL INCORPORATION OF BIOMEDICAL ENGINEERINGALGORITHMS (BISPECTRAL INDEX GUIDED OR ANESTHETICCONCENTRATION GUIDED) IN REAL-TIME DECISION SUPPORT TOPREVENT INTRAOPERATIVE AWARENESS USING AN ELECTRONICANESTHESIA INFORMATION MANAGEMENT SYSTEMbyAMY MELANIE SHANKSMay 2015 Advisors: Drs. John M. Cavanaugh and George A. Mashour Major: Biomedical Engineering Degree: Doctor of Philosophy Background: Intraoperative awareness with explicit recall (AWR) is a fearedcomplication of surgery that can lead to significant psychological distress. Several largeprospective trials have been completed comparing two methods of monitoring anesthetic depth[minimum alveolar concentration (MAC) or electroencephalography (EEG) monitoring using thebispectral index (BIS)] for the prevention of AWR. However, these trials were conducted inhigh risk populations, limiting generalizability. Research Hypothesis: Real-time decision support with Anesthesia InformationManagement System alerts based on a novel anesthetic concentration algorithm (incorporatingthe use of intravenous anesthetics) or an EEG-guided algorithm will reduce the known incidenceof AWR.

Protocol for the "Michigan Awareness Control Study": A prospective, randomized, controlled trial comparing electronic alerts based on bispectral index monitoring or minimum alveolar concentration for the prevention of intraoperative awareness

BACKGROUND The incidence of intraoperative awareness with explicit recall is 1-2/1000 cases in the United States. The Bispectral Index monitor is an electroencephalographic method of assessing anesthetic depth that has been shown in one prospective study to reduce the incidence of awareness in the high-risk population. In the B-Aware trial, the number needed to treat in order to prevent one cas...

Protocol for the BAG-RECALL clinical trial: a prospective, multi-center, randomized, controlled trial to determine whether a bispectral index-guided protocol is superior to an anesthesia gas-guided protocol in reducing intraoperative awareness with explicit recall in high risk surgical patients

BACKGROUND Awareness with explicit recall of intra-operative events is a rare and distressing complication that may lead to severe psychological symptoms. Candidate depth of anesthesia monitors have been developed, partly with the aim of preventing this complication. Despite conflicting results from clinical trials and the lack of incisive validation, such monitors have enjoyed widespread clini...

[Anesthesia awareness and the bispectral index].

BACKGROUND Awareness during anesthesia is a serious complication with potential long-term psychological consequences. Use of the bispectral index (BIS), developed from a processed electroencephalogram, has been reported to decrease the incidence of anesthesia awareness when the BIS value is maintained below 60. In this trial, we sought to determine whether a BIS-based protocol is better than a ...

Monitoring Depth of Anesthesia by Nonlinear Correlation Measures

Background: Monitoring the depth of anesthesia (DOA) takes an important role for anesthetists in order avoiding undesirable reactions such as intraoperative awareness, prolonged recovery and increased risk of postoperative complications. The Central Nervous System (CNS) is the main target of anesthetic drugs, hence EEG signal processing during anesthesia is helpful for monitoring DOA. In order t...

Assessment of Anesthesia Depth Using Effective Brain Connectivity Based on Transfer Entropy on EEG Signal

Introduction: Ensuring an adequate Depth of Anesthesia (DOA) during surgery is essential for anesthesiologists. Since the effect of anesthetic drugs is on the central nervous system, brain signals such as Electroencephalogram (EEG) can be used for DOA estimation. Anesthesia can interfere among brain regions, so the relationship among different areas can be a key factor in the anesthetic process...
